Before making a complaint, take steps to raise your concern directly with the member. Try working with the member to fix the problem and give them an opportunity to explain or propose a solution. If you can't reach a satisfactory outcome, fill out this complaint form.
You'll need to tell us:
- Who your complaint is about
- What you think has occurred
- Basic personal details
You'll need to attach any relevant documents as evidence to support your complaint. If there is insufficient evidence your complaint may be dismissed.
